About Eyelid Surgery Procedure
The Procedure
Blepharoplasty is a surgical procedure designed to correct sagging or drooping eyelids by removing excess skin, fat, and tightening underlying muscles. Dr. Guarnieri typically performs the procedure on an outpatient basis using local anesthesia with sedation, though general anesthesia may be used for anxious patients. The surgery usually takes 45 minutes to two hours, depending on whether the upper, lower, or both eyelids are treated.
For upper eyelid surgery, an incision is made in the natural crease of the eyelid to remove excess skin and fat and tighten the muscle. For lower eyelids, an incision is made just below the lash line to remove or reposition fat and tighten tissue. In select cases, a transconjunctival approach is used for lower eyelid fat removal without visible scarring—ideal for patients with puffiness but no excess skin. After reshaping, incisions are closed with sutures, tissue glue, or surgical tape, and the area is gently covered with gauze to support healing.
Recovery
After blepharoplasty, patients may use lubricating eye drops and cold compresses to reduce swelling and discomfort. Most return to work within a few days to a week, though strenuous activity should be avoided for at least two weeks. Stitches are typically removed after three to four days. Swelling and bruising usually subside within two weeks. Patients should avoid wearing contact lenses and eye makeup for two weeks. To protect the eyes, dark sunglasses are recommended for two weeks in bright light or windy conditions. Final results improve over several months as swelling resolves and healing completes.
Risks
Blepharoplasty is generally safe, but like all surgery, it carries risks. These include infection, dry eyes, temporary blurred or double vision, and changes in tear production. Scarring is rare and typically minimal due to the placement of incisions in natural eyelid folds. Uneven healing or permanent scarring may occur but is uncommon and can often be corrected with revision surgery. Patients with eye conditions such as glaucoma, retinal detachment, thyroid disease, diabetes, or high blood pressure are not ideal candidates.
