About the Breast Reconstruction Procedure

The Procedure

Reconstructive breast surgery is a multi-stage process tailored to the patient’s anatomy and goals. There are two primary approaches: implant/tissue-expansion reconstruction and autologous tissue reconstruction.

In implant/tissue-expansion reconstruction, a tissue expander is first placed under the chest muscle and skin. Over several weeks or months, saline is gradually injected through a valve to stretch the tissue and create a pocket for the final implant. The expander may be left in place or replaced with a permanent silicone or saline implant. A separate surgery is typically performed to reconstruct the areola and nipple.

In autologous tissue reconstruction, a flap of skin, fat, and sometimes muscle is taken from another part of the body—such as the abdomen, back, buttocks, or thighs—and transferred to the chest. This flap can be moved while still connected to its original blood supply (pedicle flap) or fully detached and reconnected (free flap). Implants may be used in combination with tissue flaps to achieve better volume. Nipple and areola reconstruction is done in a separate procedure to complete the look.

Both methods require careful planning between the breast surgeon and plastic surgeon to ensure optimal outcomes. Recovery timelines vary based on the technique used and whether reconstruction follows mastectomy immediately. 

    Recovery

    Recovery after breast reconstruction varies depending on the method used. Hospital stays typically range from one to six days. Patients often experience fatigue and soreness for one to two weeks, with full recovery taking three to six weeks. Compression garments are worn to support healing and reduce swelling. Stitches are removed within one to two weeks, and a surgical drain may be used to prevent fluid buildup—it is usually removed after one to two weeks. Most patients return to work within a few weeks but should avoid strenuous activity for at least six weeks.

    After reconstruction, patients may notice changes in breast sensation. While implants typically provide little to no sensation, autologous tissue reconstruction offers better sensory recovery. Scarring is minimal due to strategic incision placement, and results improve over time as swelling subsides. 

    Risks

    Reconstructive breast surgery carries risks common to all surgical procedures, including infection, bleeding, and complications related to anesthesia. Specific risks vary by technique:

    Implant-based reconstruction may lead to capsular contracture, implant rupture, deflation, or shifting.
    Autologous tissue reconstruction can result in fat necrosis, abdominal weakness, hernia, or mismatch between donor tissue and chest anatomy.
    Patients with medical conditions such as obesity, high blood pressure, or a history of smoking may be advised to delay surgery to reduce risk. Correcting complications often requires additional procedures. Dr. Guarnieri ensures patients understand all risks and outcomes before surgery.
